Dependent confirmation

The health system asks employees for documents to confirm their dependent is eligible to receive coverage under the health system’s benefits plan. This applies to all benefits where a dependent is named, including medical, dental, vision and life insurance.

It’s important to send the appropriate documentation to avoid your dependents’ coverage being canceled and/or a spousal surcharge of $50 per paycheck for medical benefits (the surcharge does not apply to dental or vision benefits). 

This best practice will help keep benefit costs as low as possible for employees. Ineligible dependents on any of our plans increases benefit costs, which impacts premiums.

The health system has partnered with Consova, a benefits confirmation expert, to manage the process and support employees. As part of this review, they will:

  • Contact affected employees, providing instructions to access its secure website and communications about deadlines and results.
  • Confirm that dependents signed up for coverage are eligible (for example, children and spouses meet the definitions of those terms in the plans).
  • Confirm whether the spousal charge ($50 per paycheck) should be applied to medical benefits. This charge applies when a spouse is offered medical insurance from their employer but chooses to be covered on the health system’s medical plan. This charge does not apply to dental or vision coverage.
  • Ensure confidentiality by providing a secure portal to share information electronically, following strict protocols to protect personal and proprietary information.

After you enroll in your benefits, Consova will send instructions to your preferred email (the one you designated in the enrollment website, bSwift) and home address. The date materials are due will vary.

  • IRS – for a copy of your previous year tax return; you may also call 800-829-1040. This is available at no cost and should be provided within 10 business days.

Spouse:


Your legally married spouse or common law spouse, as long as you reside in a state that recognizes common law marriage.


(Legally separated or divorced spouses are not eligible for coverage.)

A copy of your marriage certificate or a common law affidavit.

And

A copy of page one through the claimed dependents section of your jointly filed federal tax return (form 1040) or both of your tax returns if you file separately, from the most recent tax season. If married in the current calendar year, only the marriage certificate will be required. Please black out the first five digits of any Social Security numbers.


And, for those covering spouse on medical:  Complete and return the Benefit Verification Form. This form may be found at https://iverify.consova.com/. (Access information provided by Consova.)


Note: All employees who include their spouse as a dependent for medical coverage need to complete this form, regardless of whether or not you are currently paying a surcharge.

Child under age 26:

  • Your natural child

  • Legally adopted child or child in the process of being adopted;

  • Your stepchild;

  • A child of whom you or your spouse have full legal guardianship of;

  • A child who is the subject of a Qualified Medical Child Support Order (QMCSO) issued to you.

A copy of the following documents (varies by the relationship of the child to the employee):

  • Natural child: State or county issued birth certificate showing employee’s name or signed court order. If birth certificate lists employee’s maiden name, please provide marriage certificate.

  • Legally adopted child: State or county issued birth certificate showing employee’s name or signed court order. If birth certificate lists employee’s maiden name, please provide marriage certificate.

  • Stepchild: State or county issued birth certificate showing parents’ names, copy of your marriage certificate, and a copy of page one through the claimed dependents section of your jointly filed federal tax return (form 1040) or both of your tax returns if you file separately, from the most recent tax season. Please black out the first five digits of any Social Security numbers.

  • Child whom you or your spouse have legal guardianship: Signed Court Order and a copy of page one through the claimed dependents section of your filed federal tax return (form 1040) from the most recent tax season claiming the child as a dependent. Please black out the first five digits of any Social Security numbers.

  • Child who is the subject of a Qualified Medical Child Support Order: Signed court order.

Disabled child aged 26 or older:

A child who is incapable of self-support, provided the disability began before age 26.

For unmarried disabled children, age 26 or over: State or county issued birth certificate showing employee’s name or signed court order. If birth certificate lists employee’s maiden name, please provide marriage certificate.
